Admission Requirements:

You must be 16 years old or older and minimum 8th grade education
Copy of social security and Driver’s License
Background check approval
BLACK Scrubs to begin clinicals ( Scrubs are to be purchased and paid for by the student independently and the student must have their uniform on the first day.

THE COST OF TUITION is $1,025.00.

$1,025.00 includes your deposit, tuition, curriculum, books and skills lab.

It DOES NOT include the state's exam fee of $125.00.

There is a non-refundable $50.00 Application Fee.

I understand that once I submit my applicaton, pay my deposit, and I am accepted into Faith Forward's Nuse Aide Training Program, I m resposible to have my tuition paid in full by graduation and that if I do not pay fo my tuition, I will not recieve my certificate of completion or be approved to test with the state until my balance is paid in full.

Faith Forward Nurse Aide Training Academy is accredited by the Health and Human Services Commission and Approved and Regulated by the Texas Workforce Commission, Career Schools and Colleges, in Austin, Texas.

COMPLAINT POLICY:

This school has a Certificate of Approval from the Texas Workforce Commission (TWC). The TWC-assigned school number is: S5322 .

The school’s programs are approved by TWC, as well as Texas Health and Human Services Commission.

Students must address their concerns about this school or any of its educational programs by following the grievance process outlined in the school’s catalog. Schools are responsible for ensuring and documenting that all students have received a copy of the school’s grievance procedures and for describing these procedures in the school’s published catalog. If, as a student, you were not provided with this information, please inform school management. Students dissatisfied with this school’s response to their complaint or who are not able to file a complaint with the school, can file a formal complaint with TWC, as well as with other relevant agencies or accreditors, if applicable.
